Hart was hospitalized and underwent surgery after suffering "major back injuries" when his Plymouth Barracuda left the road and landed in a ditch early September.
TMZ experiences that the actor and comedian has moved to a rehab facility and is performing extreme actual physical therapy. Hart's goal is to get home "as soon as possible", where he can start outpatient care.

In audio obtained by the TODAY show days after the accident, an unnamed woman told a 911 operator, "I need someone to come get him". "So he's a lucky man and I'm so happy [he's OK]".
Patrol officers determined that Black was not under the influence of alcohol at the time of the collision.
